PROOF OF INCOME LETTER SAMPLE

The following documents are acceptable proof of income declaration:

  • 3 consecutive months of payroll statements or paycheck stubs for the current year
  • Last year’s state, federal, or tribal tax return
  • A Social Security statement of benefits
  • An Unemployment/Workmen’s Compensation state of benefits
  • A Veterans’ Administration statement of benefits
  • A federal or tribal notice letter of participation in General Assistance
  • A divorce decree, child support or other official document

Other documents on a Case to Case basis that may be deemed necessary by the Bank or Financial Institution.



SELF-EMPLOYED - Small Business entrepreneur

Documentary evidence of Proof of Income for loan purpose:

Statement of Accounts (Balance Sheet & Profit & Loss Statement)

Tax Returns for last 3 years with 12 month projections.

A statement of taxable income from a certified accountant or tax consultant; or

The most recent Notice of Assessment from the Tax Office; or

A Statutory Declaration of gross income (only acceptable for up to 3 months, until one of the previous documents is provided).

Scholarships, Grants, and Education Benefits

A verification Form or letter from source is needed. And copies of latest benefit checks if amount is not expected to change over the next 12 months

Dividends or Interest Income, including Savings Accounts

Copies of current statements, CDs, or Form 1099 from the institution, if amount is not expected to change over thenext 12 months.

Verification Form or letter from source.

Interest from Sale of Real Property

Verification Form or letter from the real estate agent, accountant, or other source including amortization table.

Copy of amortization schedule, with information to determinethe amount of interest to be earned during the next 12 months.

In most cases, a letter DECLARATION must be detached and returned with your income verification documents.

Proof of Residence: photocopy of Electricity Bill, Telephone Bill, Ration Card.
